Treatment for pigmentation and freckles

Hyper-pigmentation or dark spots on the face and body are common skin blemishes treated in our clinic. These spots are often caused by picked acne or other scars exposed to the sun. Sun exposure stimulates melanin production in our bodies.

Hormone changes can sometimes cause excess pigment, such as Melasma or a mask of pregnancy. Other spots, like Liver Spots, are commonly associated with general skin aging, typically occurring after age 40.

Laser Pigmentation Removal effectively diminishes the visibility of freckles, pigmentation, sun, and age spots. This laser precisely targets the pigmented areas without affecting the nearby skin. It heats and breaks down the pigment, removing it without harming the adjacent tissue.

Once the pigmented lesions are treated, they typically fade, dry up, or peel off over time, leading to a more uniform skin tone and complexion. 

Initiating the treatment requires an essential initial consultation. This meeting, usually lasting 15 to 30 minutes, involves evaluating your medical history and discussing lifestyle factors that may affect your skin.

The initial step of the freckle removal involves applying a numbing cream to the area to lessen discomfort. Following this, a suitable laser is selected and used in the area. You might experience a snapping feeling and some warmth during the procedure. Post-treatment, the freckles will turn darker, and the skin around them will become red and swollen. Ice packs or an air cooler alleviate heat, swelling, and redness.

It’s typical for freckles to become darker for around a week after the treatment before they scab and flake off. Generally, a recovery period of a week is needed, where swelling, redness, and freckle darkening might be observed.

Freckles removal is usually spaced at least a month apart, with one to two sessions often sufficient. However, additional sessions might be necessary in some cases.

Freckles may reappear due to sun exposure. Thus, applying sunscreen daily is essential, both before and after the skin pigmentation removal. 

  • Avoid direct sun exposure and apply artificial tanning products for one month before your scheduled laser pigment removal.
  • You are encouraged to avoid consuming aspirin, as its blood-thinning properties may interfere with the healing process.
  • On the day of your treatment, it’s recommended not to use products like hairspray, perfume, or aftershave, as these could heighten the chances of feeling a burning sensation.
  • Steer clear of waxing the pigmentation laser removal area.
  • Refrain from any other laser treatments.
  • It is prudent to avoid participating in any chemical skin treatments, whether at home or in a professional setting.
  • It is wise to avoid depilatory creams on the area set for pigmentation removal. 

Adhering to specific precautions for at least one-week post-laser treatment is advised.

  • Steer clear of sun exposure.
  • Daily moisturization with Hydra Factor K cream is advised.
  • Applying SPF 50+ sunscreen each day is recommended.
  • A cold compress for 2-3 hours after arriving home is suggested for the best comfort.
  • Opt for paracetamol instead of aspirin, as recommended.
  • Refrain from exercising or swimming in chlorinated or saltwater pools.
  • It’s advised to avoid hot showers, saunas, and steam rooms. 

The redness will improve after the face pigmentation removal. The freckles will darken initially and then lift and crumble away, revealing clear, freckle-free skin. Healing times may vary.

Redness is common after treatment and typically lasts for a few days. In some cases, redness can be prolonged and last six months to 1 year or even become permanent, although rare. Generally, the lower limbs take the longest to heal. Patients should understand that healing may take time, and patience is essential. Sometimes, the redness may take months to heal after treatment fully.

Prolonged redness is common, especially on the legs and sometimes on the shoulders, back, and other areas. Patients need to be patient and aware that this can happen. They should not worry and give their skin enough time to heal. Permanent redness is rare. Post-treatment redness should be reviewed at 1, 3, 6, and 12 months.

If the redness was not accompanied by blistering during the freckle removal treatment (see complications below), it is doubtful to be permanent, though it may still be prolonged.

Swelling can occur in the body, particularly in the upper limbs. We prefer to treat only half the arms due to this complication. The swelling can be uncomfortable and restrict movement.

Post-inflammatory hyperpigmentation, also known as laser-induced brown pigmentation, can occur due to inflammation caused by the laser and the patient’s susceptibility. It is more prevalent in individuals with lighter skin tones.

For certain skin types, preventing hyperpigmentation by lightening and possibly using steroids after treatment is essential. It is recommended to use sunscreen and avoid sun exposure after treatment.

Blistering is rare, particularly if the patient has undergone test patching at a specific intensity. In such cases, it is advised for the patient to contact their doctor urgently.

Infection is rare and typically occurs with broken skin, such as blisters. Symptoms include redness, tenderness, swelling, pain, and pus in the affected area. This is a severe complication that requires immediate treatment. Contact a doctor, even during non-business hours. Do not delay treatment for infection.

Hypopigmentation/hyperpigmentation scarring is not very common.

After freckle removal, it is not common for them to reappear on the body. However, it is essential to note that past sun damage may continue to occur and result in the appearance of freckles. Some patients may choose a top-up pigment removal after several years, although the skin usually does not return to its original freckled state. Treating the face, neck, chest, shoulders, and hands with a freckle removal laser is recommended annually. 

The level of sun damage determines the number of areas we can treat in one session. If there is severe sun damage, we may only be able to start with two areas of freckle removal laser (not on the same limb). If there is very mild sun damage (without any background actinic damage), up to 3 areas can be treated.

As the sessions progress, more areas can be treated in a single treatment as the freckles and pigmentation are lifted.

Typically, a freckle removal laser requires a minimum of 2 treatments per body area. Darker skin tones may need 3 to 4 pigmentation laser removal, especially on the face, back, and chest. The number of treatments is not limited. The laser pigment removal cycle ends when the patient is satisfied or if further treatment is impossible due to side effects or device limitations. This varies for each patient.
